Please fill out the form below in order to have your child have a speech consult. Speech consults are $60 each and are available for baby under 24 months. Please select your top 3 options that will work with your schedule. If any time will work for you please indicate that below. Once you sign up and we recieve your payment we will confirm your appointment time. You will also be sent an intake form that will need to be completed prior to your appointment.
